Job summary
The primary role of Somerset end of life Care Coordination Centre SCCC will be arranging packages of care, equipment and night support for patients who wish to remain at home for end of life care.
The post holder will be part of a small team liaising with District Nurses, Hospital discharge teams, and local hospices in order to provide a coordinated approach to delivering patient care in Somerset.

Main duties of the job
This role is unpredictable and the post holder will very often change from one activity to another as a result of interruptions,you will be supported by a Clinical Lead but will be expected to work with their own initiative within the boundaries of their role.
The service operates 7 days per week and is open from 8am to 6pm Monday to Friday and 10am to 3pm on weekends and bank holidays.
The post holder will be expected to cover weekends and bank holidays on a rotational basis with other team members.
- Process CHC Fast Track referrals following set process
- Source and book care provider and equipment as required.
- Enter all communication onto Computer database, up date records accordingly.
- Receive and process referrals for night support, updating records as required
- Communicating with Patient, family and or health professionals via email or telephone
- Maintain information and report from excel spreadsheets
- Stand in for Clinical lead at agreed meetings
- Dealing with confidential client issues
You may occasionally deal with challenging behaviour, or distressed callers.
The role requires mainly working within an open plan office with substantial VDU and telephone use every day.
