Job summary

This is a fantastic opportunity for a manager with a clinical background to work with multiple stake holders in the new and exciting Care Coordination team. This team is new and this post offers a real chance for the successful applicant to help shape its future that is currently in its infancy.

We value our clinicians highly, so development and training within this role will be offered and prioritised. These hours will be worked over Monday to Friday within the Care Coordination team. This post will also require the successful applicant to be part of the on call rota. This is a full time post.You will require excellent communication skills, working to trust values and be able provide a high standard of clinical and non clinical support across the teams.

Main duties of the job

To provide and maintain a high standard of evidence based, patient centred care. To demonstrate excellent interpersonal skills and to develop practical skills required within the speciality, whist reflecting the philosophy of care.Effectively managing teams in an integrated environment to support the safe and effective care of patients in the community adopting the home is best policy. Recognising the need for escalation to an acute bed when necessary but in discussion with key stakeholders to ensure that all community avenues have been exhausted before conveyance.

The post will require support the management of admin and clinical staff.

All enquiries are welcome to discuss this post further as this is also in its infancy but has the opportunity for the successful applicant to shape it to deliver within the new integrated service. This is a new county wide service (hub) that requires effective communication with the Hospital@Home, Rapid Response, Urgent Care Response, SWAST, Bed Hub, End Of Life, D2A, Transfer of care Hub and HUC and 111 and this is not an exhaustive list.
