Job summary

We are recruiting a complex care nurse to our existing team to deliver the West Somerset Primary Care Network's Living Better project. This is an exciting opportunity for a nurse interested in improving patient outcomes by working on a dynamic project developed to meet the needs of a changing healthcare environment.

The post will be based at our neighbourhood hub in Williton Community Hospital and is a community-based role covering the West Somerset practice's areas.

You will support patients, their families and carers to establish personalised care planning goals, encourage self-management and support independent living. You will be responsible for planning, proactively monitoring and anticipating the changing needs of patients on your caseload, co-ordinating care across all parts of the health and social care system, working alongside the expanding PCN, neighbourhood team and GP practice, being an advocate for cross organisational working.

This post will be hosted by Somerset Foundation trust as the employer. Service delivery will be managed by the West Somerset Primary Care Network and Living Better team.

The post available is for 28-37.5 hours depending on the candidates preference. We are in the enviable position of being able to provide a substantial period of shadowing the previous post holder who will be retiring.
