Join Our Team as a Housing Lead at the Urgent Care Hub!
The Urgent Care Hub, part of the Home Treatment Service, plays a crucial role in supporting admissions, discharges, and bed flow for mental health beds within Somerset NHS Foundation Trust. We are seeking a dedicated Housing Lead to provide active housing case management for individuals with mental health needs using inpatient, rehab, and step-up/step-down services to secure a housing and/or support option that will facilitate discharge.

Main duties of the job
As the Housing Lead, you'll support the discharge and recovery of people with mental health needs, by helping to secure suitable housing options for them. You'll help to prevent homelessness whenever possible by providing information, advice, advocacy and support services to people with mental health needs who are currently in inpatient facilities, and their families and carers.
Collaborating with other members of the Urgent Care Hub and staff from local district councils within Somerset, you will help sustain or secure housing options during hospital admissions or other crises.
Your role will also include supporting core functions of bed management, making you an integral part of the Urgent Care Hub team.
