Job summary
This role will see and coordinate the pathway of major trauma patients within the Trust with complex injuries. The post holder will work as an integral member of the Major Trauma Committee, and the managing multidisciplinary teams, whilst retaining strong links with the therapy services.
The post holder will be expected to provide expert assessment and advice and act in a Key-worker role for patients with Major trauma, ensuring their rehabilitation care needs are being met, signposting and completing appropriate referrals, and supporting patients and families. They shall also take a lead in ensuring the appropriate completion of Rehabilitation Prescriptions for these patients.
The post holder will also undertake a rehabilitation and physiotherapy caseload.
Main duties of the job
The post holder will be responsible for:
1) Identifying appropriate patients as early in the pathway as possible, working closely with the Trust Trauma Audit Research Network (TARN) coordinator, admitting teams in the Emergency Department and with the Trauma Coordinators.
2) Planning and prioritising their own workload.
3) Overseeing the completion of the Rehabilitation Prescription for all trauma patients with Injury Severity Scores (ISS) of 8 or more. These should be commenced within 72 hours of admission.
4) To act in a coordinating, key-working role to facilitate a smooth transition of care for major trauma patients and their families during their acute hospital stay.
5) To be responsible for identifying barriers to discharge/ recovery at an early stage and act upon these within the Multi-disciplinary Team (MDT).
6) To support the coordination of a smooth discharge from the acute setting for patients admitted with major trauma.
