Job summary

The post holder is suitable for a Registered Nurse, Allied Health Professional or Paramedic with a broad general experience as a qualified professional.

Hours: 36 per week (Three shifts per week)

Shifts: 8am - 9pm (Seven days a week, desired applicant will need to work set Fridays to meet service need)

Base: Bridgwater Community Hospital

Rapid Response is a service which aims to meet patients' urgent care needs at home by providing urgent clinical, therapy or support interventions to keep a patient in their place of residence, which is key in improving patient outcomes, preventing avoidable hospital admissions and delivering NHS strategic priorities. The service is part of the new Urgent Community Response agenda and will respond within two hours of a referral to meet the UCR national guidelines.

Main duties of the job

The Rapid Response Coordinating Centre is responsible for the triaging of referrals, telephone calls and the daily management of the Centre between the hours of 08:00-21:00 seven days a week.

The candidate will be responsible for triaging referrals on to the Rapid Response caseload, identifying the most appropriate professional to see the patient, allocating visits and liaising with clinical staff.

The candidate will be responsible for signposting patients and/or their carers/relatives to the provision of accurate healthcare information, self-care/health improvement advice or, where applicable, referral to the appropriate level and location of care, in order to meet the holistic and therapeutic requirements of the patient/carer.

The candidate is responsible for the management and clinical supervision of junior staff and the facilitation of data collection, audits and quality improvement to the service.

The successful candidate will act as an autonomous practitioner, within their boundaries of clinical practice with the freedom to make clinical decisions based wholly on their experience, knowledge and analytical skills used during the telephone triage and remote assessment process with calls of an urgent nature to patients and/or their carers/relatives resulting in a safe and appropriate clinical decision that reflects their clinical need. They will require excellent communication skills and have the ability to work in a multidisciplinary environment under pressure
