Job summary

We're recruiting for our Community Rehabilitation Service in the Taunton and West Somerset Neighbourhood, providing physical rehabilitation to our community patients across Taunton and West Somerset. We are currently looking to expand our community links working in a more collaborative way with patients, other agencies and the voluntary sector within our local 'neighbourhood'.

You will be working from one of two bases within the Neighbourhood - one at Minehead Hospital where there is a bedded rehab unit, as well as community patients to be seen. The other, based on the edge of Taunton near the M5 Junction, covers community work only. You will be expected to provide cover across both areas.

The team consists of OTs, Physiotherapists, Clinical Specialists, Assistant Practitioners, Apprentices, Rehabilitation Assistants and a Pharmacy Technician. This position will provide the opportunity for you to develop holistic cognitive, environmental & functional assessment skills in the community across a variety of conditions. You will also develop your clinical supervision and delegation skills, rehabilitation and clinical skills, deepen your knowledge base and strengthen your clinical reasoning in a structured, supportive environment.

Main duties of the job

The Post Holder will:-

  • Provide a high standard of Occupational Therapy within the Rehabilitation services.
  • Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments.
  • To provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT.
  • Provide support, guidance and training to Senior Rehabilitation assistants, Rehabilitation Assistants and students.
  • Responsibility for a caseload of complex patients within both the community and the community hospital bedded environments.
  • Act as Clinical Supervisor and undertake Managerial Supervision, including Appraisal, for Band 5 Occupational Therapists and Rehabilitation Assistants.

To accept clinical responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time. This may primarily be community based but will require cover for other patient groups during times of staff absence for holidays, sickness, etc. Within the working day, there can be frequent interruptions to the work pattern

To communicate effectively with patients and carers to maximise rehabilitation potential and to ensure understanding of their condition. Communication skills of persuasion, motivation, explanation to encourage patients to undertake their treatment programme.

To use communication tools (verbal and non-verbal) to gain valid informed consent with patients where there will often be barriers to effective communication e.g., loss of hearing, altered perception, expressive and receptive dysphasia, pain, fear. To have the capacity to work within a legal framework with patients who lack capacity to consent to treatment eg. those with cognitive or speech difficulties.

To evaluate patient progress, reassess and alter treatment programmes if required

To formulate and deliver an individual Occupational Therapy treatment programme based on extensive experience and a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and a broad knowledge and understanding of treatment skills.

To be aware of and appropriately manage sensitive and/or complicated information. To deal sensitively with patients who may have challenging behaviour, have high levels of anxiety and aggression caused by pain, dementia, limited mobility, terminal illness, degenerative disease etc.

To be responsible for maintaining accurate and comprehensive patient treatment records in line with Trust procedures and Royal College Of Occupational Therapy (RCOT) standards of practice.

To write concise, informative reports detailing rehabilitation outcomes and levels of disability for medical staff, employers, solicitors, and Education Health Plans

To work with fellow Senior Staff and Managers to ensure a high standard of clinical care in line with national and local standards and the RCOT code of professional conduct.

To maintain personal clinical professional development (CPD), by keeping abreast of any new trends and developments and incorporate as necessary into the workplace.

To take an active role in the in-service training programme by attendance and deliverance of presentations and training sessions at staff meetings, peer review, in service training sessions and by attending courses and practising reflective practice.

To participate in the staff appraisal and supervision scheme and to be responsible for complying with your agreed personal development programme to meet set knowledge, skills, and competencies.

To undertake the measurement and evaluation of your work and current practices through the use of evidence-based practice projects, clinical audit and outcome measures either individually or within the different teams.

To contribute to the induction and training of students and staff.

To contribute to audits and research as required.

  • Diploma/Degree/Masters in Occupational Therapy
  • Registered with HCPC as an Occupational Therapist
  • Evidence of CPD maintained in a portfolio including attendance at relevant post grad courses.
