An exciting opportunity has arisen for a Registered Mental Health Nurse, Social Worker, or Occupational Therapist to join our Home Treatment Service in Somerset.
Due to additional funding, we are developing a First Response Service in Somerset, to provide telephone support and triage to individuals presenting in mental health crisis. This new team will sit under the umbrella of the Home Treatment Service and will work closely with the Home Treatment teams, with staff being able to spend time working across both services.
You will act as a key member of the multi-disciplinary team, liaising with statutory services and other agencies. You will be part of a dynamic team, who are responsible for ensuring rapid community assessments of mental health and risk. You will have sound assessment skills and provide brief interventions to clients as an alternative to admission.
This expansion will develop capacity for the Home Treatment service to provide additional treatment and care to patients at home, as an alternative to hospital and contribute to the provision of acute community mental health care in Somerset.

Main duties of the job
We recognise that recovery from acute mental illness is best met within a persons own home, so we strive to provide safe, intensive mental health treatment and intervention at home as an alternative to admission to psychiatric hospital.
You will be a key member of a skilled Multi-Disciplinary Team who will offer a wide range of psychological, medical and social interventions to support recovery
As well as working with a clinical caseload and their families, the home treatment team gatekeep admissions to and facilitate early discharge from psychiatric inpatient settings.
The service has seen many positive developments in a short period, which we continue to play an important role in- supporting individuals in a Step- up House as an alternative to hospital, and having close working links with the Mindline phone and Crisis Safe Space services in the county. These collaborative and integrated working models are part of an exciting future for the people of Somerset in what is available to support their Mental Health.
This role will involve flexible working across a 24-hour shift rota (days and night shifts) in a 7 day a week service.
