Job summary

The post holder will lead on the continued development of the Personalised Care and Support (formally Living with and Beyond) programme across Somerset NHS Foundation Trust. The post holder will continue the roll out of this programme of personalised care across the Trust within Cancer, influencing the cancer workforce to adopt these principles becoming business as usual. This role will facilitate and work with team across cancer services to embed each intervention within PCS, including Holistic Needs Assessments (and Care Plan) Treatment Summaries, Health and Wellbeing Events plus Personalised Stratified Follow-up.

Main duties of the job

The post holder will ensure the delivery of efficient and effective planning and implementation of the personalised care agenda:

  • Accountable and responsible to the Lead Cancer Nurse.
  • Responsible for leading on the delivery of the specified Cancer Personalised Care and Support (PCS) programme (including Holistic Needs Assessments, Health and Wellbeing Events, Treatment Summaries and Risk Stratified Pathways).
  • The post holder will work with cancer multi-disciplinary teams on the delivery of the PCS agenda.
  • Support service leads in the delivery of service transformation and embedding of risk stratified and patient initiated follow up.
  • Liaise with clinical and administrative teams to produce appropriate processes to support the development of the services.
  • Support services and teams, liaising with stakeholders with implementation of Treatment Summaries
  • Liaise with primary care partners with regards to service development and potential changes within primary care.
  • Raise awareness to support the delivery of the project.
