Our District Nursing team cover the Sedgemoor area. We collaborate with GP surgeries and play a pivotal role in the primary care network. Our holistic approach involves working closely with Living Better Nurses, Village Agents, and numerous volunteer groups. We are proud to have strong partnerships with Adult Social Care, Older Persons' Mental Health, and Rehabilitation services.

Main duties of the job
You must be aRegistered General Nurseand community experience is desirable but not essential as full support will be given.
Responsibilities include: End of life care, Venepuncture, medication administration, wound care, leg ulcer management, catheter care, bowel management, continence assessments, diabetes management
Shift patterns include: 8am-4pm, 9am-5pm, 10am-6pm, 2pm-10pm

Your responsibilities will include assessing both simple and complex care needs, developing, implementing, and evaluating care programs, and delegating tasks to other members of the community nursing team when appropriate. You'll also refer patients who require a comprehensive care needs review or community care assessment to the District Nurse Team Leader. In the absence of the District Nurse, you'll share responsibility as the joint coordinator of caseload management.
