Job summary

The Assertive Outreach Team (AOT) in Bridgwater are inviting applications for a Community Support Worker to join our close knit multidisciplinary service.

This is a great opportunity for you to work in a growing Assertive Outreach Service in Somerset.

The team provides mental health support to individuals with complex, severe and enduring mental health difficulties in the Bridgwater area.

The Assertive Outreach Service focuses on supporting individuals who historically have struggled to engage with standard community mental health care and are at high risk of relapsing or admission into inpatient services.

Main duties of the job

You'll be working with individuals in the community within a shared caseload with the support of the rest of the team. You will also have a key role in supporting other members of the team in the development and delivery of mental health care in the community.

Provide support with daily living; "living ordinary lives" by agreeing a support action plan with service users. This plan is based on support identified by the care co-ordinator in the care plan.

The team are committed to a recovery approach and have the ability to be flexible to support each other.

Access to own private transport and willing to transport patients as required.
