Job summary

We are aCommunity Therapy team, working with Physiotherapists, Occupational Therapists, Rehab Assistants and Pharmacy technicians.

Our aim is to help the local population maximise their abilities to improve their quality of life. We currently have an exciting opportunity for an Occupational Therapist (Band 5) to be part of the specialist community rehabilitation therapy team in Somerset.

We are changing as we move to an innovative neighbourhood model as part of an Intermediate Care Service. We have an emphasis on prevention, early intervention at the initial point of need and long term self-management to enable patients to live better lives in their home environment.

We see people in a variety of settings, led by their goals but to include: their own homes and local community, our community hospitals and our local care homes.

Main duties of the job

The Occupational Therapist role will work mainly in Crewkerne Community hospital Ward with opportunities for a community caseload within the South Somerset West areas of our Community Rehabilitation Teams, specifically covering PCNS in Ilminster, Crewkerne, Chard, Langport, South Petherton and Martock.

You will play an integral part in the Community Rehabilitation team, managing a complex community case-load assessing and treating patients either in a clinic setting or a variety of residential settings. These patients may present with a broad range of conditions including Orthopaedics, frail elderly, post-surgery and neuro long-term conditions.
