Job summary

Are you a enthusiasticand dynamic registered nurse with a passion for providing safe, effective, and personalised care in patient's own homes?

The Bridgwater Neighbourhood District Nursing Team are looking for nursing colleagues to join our integrated team, to deliver patient care in the community! Our team cover the geographical area across Bridgwater, North Petherton and surrounding areas.

You will be joining a large, friendly team we share a passion for providing patients with the very best care in their own homes, supporting early discharges from hospital, and helping to reduce unnecessary hospital admissions.

The service covers 08:00 - 22:00 including weekends and bank holidays on a rota basis. Shift patterns include: 8am-4pm, 9am-5pm, 10am-6pm, 2pm-10pm. We are open to hearing from applicants who are looking for either full or part-time hours on a permanent basis.

Main duties of the job

The role is based within a dynamic team, integrated with older peoples mental health, the community rehab team, rapid response, hospital @ home and the primary care network.

You will be responsible for:

  • end of life care,
  • venepuncture,
  • medication administration (including IV's) wound care,
  • leg ulcer management,
  • catheter care,
  • bowl management,
  • continence assessments,
  • diabetes management
  • delivery of the hospital @ home initiative.

You will be working under the direction of the DN Team Leader to be responsible for the assessment of both complex and simple care needs, the development, implementation and evaluation of programmes of care and delegation of workload where appropriate to other members of the community nursing team.

You will also be referring patients who require a complex review of care needs or community care assessment to the District Nurse Team Leader, you will share responsibility as the joint coordinator of caseload management in the absence of the District Nurse.
