Job summary
An exciting opportunity has arisen for a confident, enthusiastic and motivated individual to work within the Outpatients Team at Somerset Foundation Trust (SFT) on a 12 month fixed term/secondment post.
You will work with clinical service managers to organise the outpatients outsourcing programme for patients at both Musgrove Park Hospital (MPH) and Yeovil District Hospital (YDH), and assist the coordination of a comprehensive patient centred outpatient service ensuring continuity in achieving excellence in patient care within local and national time frames.
Main duties of the job
The post holder will be an excellent communicator with colleagues across multiple teams both inside and externally to SFT as well as patients and their families.
The role requires excellent coordination and time management skills as well as a good working knowledge of using IT systems and spreadsheets. The role will require strong negotiation skills to support patient understanding of the benefits of receiving their treatment outside of SFT.
Although this role sits within the Outpatients team, the role itself is an independent one requiring the post holder to be able to use their own initiative and work independently.

- Communicate effectively with bookings supervisors and medical secretarial teams on all matters concerning their specialty pending lists
- Communicate effectively and in a timely fashion with patients/parents/carers as the need arises. Use reassurance and negotiation skills with tact on a daily basis over the telephone
- Overcoming barriers to understanding
- Manage real time the outsourcing spreadsheets ensuring they are accurate and reflect the current transfer situation
- To liaise with appropriate colleagues in the selection of patients for transfer. To produce patient transfer lists and share with the outsourcing provider. To contact potential transfer patients
- To prepare appropriate reports to assist in the triage of potential patients for transfer and assist external triage teams where appropriate in reviewing this information
- During the triage process, to update Trust IT systems with relevant information and accurately record decision making as to suitability for transfer within the triage report
- To produce the weekly outsourcing report update on outsourcing activity and email out to all relevant staff
