Nursing Assistant Practitioner
This is an exciting new opportunity to join the Rapid Response team as a Band 4 Nursing Assistant Practitioner!
The role involves working alongside the Healthcare Support Workers, Therapy APs, Registered Nurses, Paramedics, Occupational Therapists and Physiotherapists to support and prevent unavoidable hospital admissions to service users living in their place of residency.
The role involves covering a wide patch of patients across the West of the county, with the potential to cross cover in the East of the county.
We are looking for a full time position for a fully qualified Band 4 practitioner, however, part time positions will be considered.
Shift times will vary between 8am and 9pm 7 days a week, including a choice of 7.5 hour shifts or 9.5 hour shifts.
Bases available are Harrison House Taunton, or Bridgwater Community Hospital.
Main duties of the job
- Responsible for the holistic assessment of both complex and simple healthcare needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team.
- Working at a level above that of the healthcare support worker and have a more in-depth understanding of factors that influence health.
- To work closely with the relatives, enablers, and carers to holistically support the service user at home.
- The post holder will work as part of a collaborative team and join multi-disciplinary team meetings where necessary.
- To support service users on either discharge from Hospital, or Admission avoidance pathways.
- To support the registered practitioner in the Urgent Care Response assessments.
- To support the upskilling of support workers and students.
