This role is to assist and facilitate patient flow and a timely discharge process for patients., from point of admission to the point of discharge to their own home or alternative care facilities.
We offer a 7 day service including Bank Holidays 0830-1630 (no night shifts).

Main duties of the job
To assist and facilitate Patient Flow/ wards and patients in a timely discharge process, from point of admission to point of discharge to the patient's own home or alternative facility.
- Ability to work as part of a team liaising with inter-disciplinary team members regarding the planning of discharge for patients and timely early identification of potential discharge delays. To assist in communicating the process but not considered responsible for making the recommendation request.
- Liaise with other agencies, patients, family and/or carers in order to provide effective communications to assist with timely discharge of patients.
- In conjunction with therapy and ward staff, assist in placing appropriate patients on the Intermediate Care waiting lists, completing Pathway referrals (sections appropriate to grade), knowledge of level of detailed required and completing Social Work Referrals
- To be aware of various documented guidance from national and local level regarding discharge and options of pathways and where to source these from. Promote usage of this documentation at ward level.
- Work in conjunction with the Wards and IDT colleagues to coordinate timely and effective discharges. Ensure patients are ready by assisting with all requirements on the discharge checklist. i.e. restart package of care, Transport and completed checklist.
