Job summary
Applications are invited for an RCS accredited Senior Clinical Fellow in Bariatric & Upper GI Surgery commencing October 2023 for a period of 12 months.
You will be exposed to a wide range of bariatric procedures, principally laparoscopic gastric bypass, banding, sleeve gastrectomy, intragastric balloon and revisional bariatric surgery.
The current unit NHS volume is several hundred procedures per year, and it is expected that you will be capable of independent practice by the end of the post.
Somerset NHS Foundation Trust is an International Centre of Excellence, the Level 1 Bariatric Surgery Centre for the Southwest region and has an established Bariatric Fellowship programme with Royal College of Surgeons of England approval. As well as training in theatres, you will also be exposed to the bariatric MDT and to outpatients. You will be encouraged to become involved in the units research activity and will contribute to the Middle Grade General Surgical on-call rota (non-resident) which is supported by both CT and F1 on-call tiers.
Main duties of the job
- Work as a member of the multidisciplinary Upper GI & Bariatric team to provide specialist Upper GI & Bariatric services for outpatients and on the wards.
- To work flexibly with the 5 Consultants to ensure cover for wards, outpatient clinics and operating lists.
- Play a full part in the provision of surgical services, potentially including scheduled out-of- hours work and resident/non-resident on-call duties on a rota basis with colleagues.
- Be involved in the development and planning of surgical services within the Trust.
-Undertake the administrative duties associated with the care of patients.
- Participate in audit examining activity, quality and cost of the surgical service. Participate regularly in medical management and audit meetings.
- Take part in the appraisal processes of the Hospital and its clinical governance activities.

Trainees
The department currently has 1 peri-CCT Bariatric Fellow, 1 SpR, 1 Trust registrar (this post), 1 ST1/2, 1 SHO Clinical fellow and 3 F1 trainees.
Equipment and Facilities
There are 5 General Surgery operating theatres with one dedicated emergency theatre. We have an integrated laparoscopic theatre (OR1) and we have video links from the operating theatres to the lecture theatre in the Academy for training purposes. We have facilities for laparoscopic ultrasound and laparoscopic choledochoscopy. There are 4 inpatient surgical wards and a 12 bedded ITU / HDU complex is located directly opposite theatre. Day surgery has 4 additional operating theatres, and the endoscopy unit has 3 suites.
The department has 3 nurse specialists (2 bariatric and 1 UGI) and the bariatric service is supported by a full MDT of dieticians, endocrinologists, anaesthetists, psychologists and gastroenterologists. The department is supported by interventional and cross-sectional radiologists with facilities for on-site CT and MRI, together with ERCP.
The Department has facilities for word processing, data processing, research and audit.
Postgraduate Education
Surgeons of all grades are required to undertake continuing medical education, take part in the local teaching programme and maintain their own interests and standards. Attendance at teaching sessions and audit meetings is expected.
Workload
General Work The surgical work of the Trust includes all District General Hospital surgical specialities including general surgery, vascular surgery, gynaecology, maxillofacial surgery, ENT, ophthalmic surgery, urology and orthopaedics as well as specialist surgery such as bariatric surgery and spinal surgery
Upper GI and Bariatric Surgery Service
This is a very busy Upper GI unit which undertakes the usual Upper GI work of a District General Hospital but it is also the tertiary referral Bariatric unit for the South West region (population 5.5 million), undertaking over 200 NHS bariatric surgical procedures per year. The vast majority of operating on the unit is of a minimal access nature and it is carried out on both an inpatient and day case basis. In addition to scheduled operating lists in both main theatre and day surgery, there are 3 outpatient clinics and 3 Endoscopy lists a week (including ERCP).
The Upper GI surgical service is focused on benign work (surgery for upper GI malignancies is undertaken in Bristol), although the unit provides support to the gastroenterology department and MDT in the diagnosis and pre and post-operative management of malignant cases.
Emergency Surgery
The majority of urgent General Surgery is listed for the dedicated all-day Emergency Theatre and is largely consultant led. Night-time operating is at consultant-level only.
Audit & Research
Review and Education
All surgeons and associated staff attend the monthly Directorate meeting, which is held on the same day as the audit session. The audit half day is currently organised on a rolling programme so that a different set of theatre sessions is affected each month. Audit topics are planned and published yearly in advance
Research
The Bariatric unit has an international reputation for research in this field.
