Are you a newly qualified ACP? Looking for your next challenge within Acute Medicine? As an Advanced Clinical Practitioner you will be working independently as an Advanced Clinical Practitioner within the Division of Acute Medicine and Same Day Emergency Care.
You will be expected to provide advice, guidance and support to the team within the Acute medical division. (Medical and Nursing)
You will be expected to deliver clinical duties but will be provided regular CPD and development opportunities
You will be able to work autonomously across the medical take. Contributing to the medical plan and facilitating discharge / sign posting as appropriate.
This will have a Same Day Emergency Care focus but you will be expected to provide cover to their commensurate level within the Acute Medicine catchment (ward based, Emergency Department)
Main duties of the job
To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the Acute Medicine service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses,
To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required. Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.
To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.
To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care.
