Job summary

An exciting opportunity has arisen for a highly motivated individual to join the Emergency Department at Cambridge University Hospitals as a Patient Flow Navigator.

You will have excellent communication and co-ordinating skills, be able to use initiative and have the ability to work under pressure while working with Clinical and Operational teams to ensure that patients are actively managed through Emergency patient pathways.

To work closely with clinicians, nurses, and all members of the Multi-Disciplinary Team (MDT) across all specialties to ensure that patients are actively managed through Emergency patient pathways, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

Main duties of the job

As an integral member of the Emergency Department team the post holder will work with other members of the Emergency Department operational and clinical teams as well as liaising with the Site Team and Specialty Teams to:

  • Coordinate patient flow by working alongside the ED Senior Nurse in Charge (219) and Emergency and Surgical Assessment Units (EAU/SAU) Nurse in Charge shift manager to utilise bed and chair capacity
  • To highlight long waits and delays through the escalation process and support mitigation actions
  • To collect, record and communicate accurate, timely and complete information to support operational and clinical needs and to prepare for 3 hourly hub meetings.
  • Maximise patient flow to deliver 4 hour access standards and relevant clinical emergency medicine standards e.g. 15 minute ambulance offload, time to be seen by clinician.
  • Support the operational team in maintaining a Trust wide bed state by ensuring the Trust's electronic patient system (EPIC) is up to date.
  • Facilitate the patient transfer to the wards by ensuring senior clinical decisions are made and notes are up to date on EPIC.
  • To manage the track board and escalate any deviation from (IPS)
  • To book hospital transport for patients who meet the clinical criteria
