A fantastic opportunity has arisen for a motivated Specialist Support Frailty Nurse to join our highly skilled & experienced Early Intervention Team at Cambridge University Hospitals.
About usThe Early Intervention Team (EIT) is a very proactive and innovative multidisciplinary team of Frailty Specialist Nurses, Discharge Planning Specialist Nurses, Occupational Therapists and Physiotherapists. The Early Intervention Team has been well acknowledged for providing proactive Frailty screening, and Admission Avoidance where appropriate. EIT Frailty Nurses identify frailty in patients over 75 years old and initiate Comprehensive Geriatric Assessments for patients admitted to the Emergency Department, Urgent Treatment Centre, Same Day Emergency Care and inpatient wards. The Team provides an excellent opportunity to develop quality improvement and service development skills as part of the service expansion.
About the roleWe are looking for an innovative and motivated team player, working collaboratively with other members of the multidisciplinary team, including our community partners, to provide a holistic, patient-centred frailty service.
Main duties of the job
You will support patients and their families/caregivers by comprehensive screening and assessment. You will provide a specialised assessment in order to inform clinical practice, avoiding unnecessary harm and informing clinical management to patients living with frailty syndromes. In addition you will help prevent the unnecessary admissions of patients into acute care or to initiate early discharge planning
The post holder will be responsible for providing a high standard of assessment, advice, and evaluation of patients to formulate ongoing plans of care for patients both those admitted to the hospital and those discharged into community settings. This may include patients with diverse presentations and complex physical and psychological conditions.
The role will allow development of specialist knowledge of ED, Frailty, and Discharge Planning specific assessments, with an opportunity to evaluate practice and audit outcomes. Development opportunities will include continuing professional development, links with community services, contribution to operational group meetings, and striving to meet our national and local guidelines.
The successful applicant will be expected to contribute to seven-day working, between the hours of 8 am to 8 pm based on a rota system.
