Job summary

We are looking for a highly motivated individual to join the Cancer Support Team.

Navigators offer practical and emotional support to patients and their families. They meet patients early in their pathway and are a point of contact throughout. They will offer patients holistic needs assessments throughout their treatment and follow up, helping to ensure concerns are addressed early. It requires excellent communication skills, as you will be liaising daily with all health care staff.

You must be able to work autonomously and build relationships with the site team you are attached to. Navigators are able to offer practical support and advice,

Main duties of the job

The main duties of the role are to support patients with their non clinical needs and be a key player in the implementation of Personalised Care.

Assist the Cancer Support Team and healthcare professionals to implement Holistic Needs Assessment and End of Treatment Summaries.

The Cancer Navigator will support people to navigate the complex health and social care system and carry out holistic needs assessments and care plans with patients.

Act as a main point of contact for patients, carers and wider healthcare professionals, ensuring the joining up of services, coordination, information and communication between all creates a seamless experience of care.

  • Good standard of general education to GSCE level or equivalent.
  • Educated to A level or NVQ level 3, level 3 BTEC with knowledge of Health & Social Care.

Desirable

  • Coaching qualification.
  • Foundation Degree/Post Graduate Diploma.

Experience

  • Extensive patient or public facing experience in a care or support role in a health care setting.
  • Prioritising, planning and organising own tasks to meet competing deadlines.
  • Experience of working as part of a team and individually.
  • Experience of working within guidelines, policies and protocols.
  • Experience of dealing with difficult situations and confrontations with both colleagues and service users.
  • Proven experience in data entry and associated documentation.
