Job summary
The Learning from Deaths team have an exciting opportunity for a caseload manager. This post is for 22 months covering a secondment and is organisational wide.
The LfD caseload manager will support the LfD Clinical lead and the Lead nurse for mortality for the delivery of the LfD service across the organisation.
The post holder will be responsible for ensuring the smooth running of the service, including ensuring that death reviews are carried out as per national and local priorities, allocating reviews to the reviewers and following up review findings for peer review. In addition, the post holder will ensure engagement with clinical teams involved in patient care to ensure themes and learning are progressed. Tact and diplomacy are required for this role and the ability to deal with situations that may be sensitive.
There is a strong link with the patient safety team and the quality improvement team using themes from reviews to improve patient safety and quality.
This post requires an individual with clinical knowledge and the ability to work independently and as part of a varied team.
Main duties of the job

In addition, the post holder will ensure that themes and learning are feedback to the clinical teams involved in patient care. Tact and diplomacy are required for this role and the ability to deal with situations that may be contentious.
The post holder will need to demonstrate a willingness to participate in projects and improvement groups using thematic analysis from LfD reviews in order to improve the quality of care we provide for our patients.

- In conjunction with the Lead nurse for mortality, ensure that policies, guidelines etc are up to date and include national and local processes.

- Facilitate a regular senior review meeting to enable the review team (and invited clinicians as needed) oversight of all cases identified by the clinical reviewers as poor care or very poor care.
- Working with the LfD administrator, collate a regular portfolio of cases where poor care has been identified by a reviewer to provide the review panel with a decision making framework for categorising the cases according to local definitions.
- Where a case is highlighted as requiring decision making against the more than likely 50% preventable due to problems in care following the review panel, liaise with the Patient Safety team to enable timely handover of the case and its associated records.
- Attend organisational mortality and morbidity meetings to feedback LfD reviews inclusive of good and poor care for learning and dissemination as well as celebrating good aspects of care.
