An exciting opportunity has arisen to join the newly established West Suffolk Virtual Ward and be part of a friendly, supportive, and dynamic multi-disciplinary team.

This is part of the national Virtual Wards Programme, an exciting transformation in how care is planned and delivered in England, building on the innovative Covid-19 Virtual Wards work over the first two years of the pandemic.

In West Suffolk our Virtual Ward provides specialist led care for a variety of patients including those with respiratory conditions, frailty, acute kidney injury and heart failure, but with anticipated expansion in areas such as women's, childrens, and surgical patients. This care is provided to them in the comfort of their own homes using an advanced personal remote monitoring platform, enabling them to safely leave hospital sooner.

There will be an educational aspect to this role to keep up with current practice recommendations, and to continuously improve the quality of the service.

We are looking for a talented and innovative Occupational Therapist to join the team and make this role their own as the Virtual Ward continues to expand and develop.

Main duties of the job

This post is suitable for a band 6 occupational therapist with a keen interest in working in community and acute with a varied caseload. As a key member you will support the virtual ward team within your occupational therapy role, with close links formed across the community health and social care teams and other locality based organisations.

The Virtual Ward team includes community nurses, specialist nurses, advanced care practitioners, consultants, therapists, healthcare assistants and a range of other health and social care staff.They work together to assess and provide support enabling early discharge from hospital or avoiding admission to hospital.

You will have an autonomous role but also be part of a close team which highly values the role of the occupational therapist as part of their team. You will receive support and development through both the virtual ward team and the adult occupational therapy service at West Suffolk NHS Foundation Trust.

We are keen to develop our staff through provision of training and development and there will be the opportunity to progress clinical, and leadership skills in this role.

Job responsibilities:

  1. Clinical and Professional

1.2 To manage a caseload of patients which require a variety of complex assessments and interventions under the following pathways - heart failure, acute kidney injury, intravenous antibiotics, respiratory, with the expansion of pathways in the future. To be involved in the development of future pathways

1.3 To participate in 7 day working if required, extended weekday working hours and weekend working rotas depending upon the need of the service

1.4 To assess and treat patients with disabilities and complex needs who would benefit from a co-ordinated multi-disciplinary assessment, rehabilitation, and onward referrals

1.5 To undertake appropriate assessment programmes to determine patient needs and prioritise patients treated according to need.

1.6 To consider all the elements and select the appropriate course of action, to achieve the best possible outcome by processing a range of information with each individual patient, who may have complex physical, cognitive and mental health needs.

1.7 To provide treatment to facilitate maximum independence by processing and interpreting a range of information and using the techniques that are appropriate to occupational therapy.

2.2 To ensure excellent communication ie with verbal and written handovers, ward rounds, MDT meetings and case conferences to ensure effective communication in regards to patient progress and treatment plans. At all times, occupational therapists need to respect confidentiality whilst giving and receiving complex clinical and personal information.

2.3 To maintain all necessary communication using the relevant patient/customer documentation IT system. To ensure other accessible IT systems are also accessed as essential in order to inform decision making and safe practice.
