Job summary

In September 2016 health and care partners (WSFT, Suffolk County Council, Norfolk and Suffolk Foundation Trust and Suffolk GP Federation) formed the West Suffolk Alliance and committed to work together to improve the health and care system in West Suffolk for all people whether they be a child, part of a family or a single adult. The focus of the Alliance is on people and places based around geographical localities. The Alliance aims to shift from working as individual organisations towards being a fully integrated single system, with a shared vision, clear local priorities, able to both provide an improved service for people in West Suffolk and to tackle the sustainability issues faced by the system together. In 2017 the partnership was awarded the contact for community healthcare services, hosted by WSFT to support delivery of the Alliance ambitions and strategy for integration. In 2021 the Alliance partners agreed a new operational and governance structure to drive improved integration between adult health and social care and introduced a new Director of Integrated Adult Health and Social Care reporting to the West Suffolk Foundation Trust and Suffolk County Council to progress the development and implementation of this.

The post of Integrated Locality Lead is a pivotal one in the new integrated operational structure to further develop the locality based integrated teams and locality infrastructure.

Main duties of the job

The main purpose of this role is to lead the development of integrated operational delivery of adult health and social care in the Newmarket and Mildenhall and Brandon localities to deliver better outcomes for people to live as independent lives as possible. You will be responsible for leading on specific clinical/care governance activity within the INT, which will include the oversight and monitoring of standards, responding to complaints, staff support, learning, development, supervision, and the implementation of policy and clinical guidelines. The Integrated Locality Lead will be expected to deliver transformational as well as operational change to ensure the priorities of the Alliance partners and of the locality are delivered effectively.

The post holder will work closely with transformational colleagues and professional leads to deliver these changes. You will promote a culture of value for money, outcome-based customer / patient focus and continuous improvement.
