Are you looking for a new challenge and keen to develop a service using advanced practice skills?

The Early Intervention Team (EIT) are looking for an ambitious, multitasker, experienced ACP to work with us in developing our admission prevention service. The role will be working in the community reviewing patients in a crisis working alongside: therapists, nurses, social workers and support workers.

Main duties of the job

The practitioner will be able to provide a holistic medical/ clinical assessment of patients, such as: post non- injury fall, reduced mobility, chest and urine infections, increased confusion and able to diagnose and prescribe to enhance the EIT intervention.

The team are working to develop the Urgent Community Response guidelines and support more admission avoidance work supporting the ambulance service and 111.

The postholder will also develop pathways and clinical links with primary and secondary care colleagues. You will work closely with a number of teams to develop new ways of working.

Full or part- time considered. The ACP service will run 7 days a week 8:30- 21:00.

JOB PURPOSE:

  • To practice as an Advanced Clinical Practitioner (ACP) within own sphere of competence and agreed acceptable limits of practice
  • Responsible for assessing, diagnosing, interpreting investigations and treating patients within the community.
  • To work as part of the multi-disciplinary team in providing quality evidence-based health care to meet the immediate needs of the patient for people at risk of hospital admission or to support an enhanced level of care after hospital discharge
  • To be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, without supervision, and to teach other staff
  • To provide day-to-day clinical leadership to the team, participating fully in the delivery of healthcare provision.
  • To provide clinical expertise for developing and enhancing the service and will demonstrate advanced skills and competencies and ensure that evidence-based practice is inherent in all aspects of care and treatment.

KEY TASKS

Clinical Responsibilities:

Practice autonomously as an ACP using collaborative and independent judgement to assess, investigate, diagnose, plan, implement, manage, and evaluate clinical nursing and medical care in the management of patients.

You will be responsible for the initial assessment and treatment of patients presenting with undifferentiated or undiagnosed conditions and make professional autonomous decisions regarding treatment within you scope of practice

Discuss highly complex medical issues appropriately with parents/carers using language that aids understanding.

Negotiate sensitively with parents/carers where consent is required for treatment that is vital for the welfare and survival of the patient and where there may be barriers to understanding through fear, anxiety or lack of knowledge, language difficulties or religious beliefs.

Work within agreed Local Trust and National Policies, Procedures and Guidelines

Following a clinical assessment and examination of the patient the ACP will make clinical management decisions utilising evidence-based practice where possible. This information will then form the basis of the clinical management plan, requesting further diagnostic and/or therapeutic interventions in line with already existing trust guidelines.

Whilst making a holistic assessment of healthcare needs the opportunity of disease risk management will also be recognised and discussed

To provide holistic patient care within the unscheduled and scheduled care setting in the form assessment and diagnosis, and by initiating treatment plans whilst the patient awaits senior review.

Prescribe medicines independently (where legally permitted) or uses PGDs to review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice to optimise treatment

Develop a plan of care, which is individualised, and patient focussed.

Communicate current health care measures and strategies for health education to the patient in both a realistic and therapeutic manner

Refer patients appropriately to other healthcare professionals as required

Ensure GPs and appropriate healthcare are involved in patients care.

Leadership:

Liaise with on an on-going basis, informing them of patients management plans as appropriate

Plan and co-ordinate the patients assessment, treatment and discharge/admission

Demonstrated effective leadership at all times

Clinical Governance:

Participate in audit, clinical governance and other clinical meetings where the outcomes and recommendations have the potential to improve the quality of care; give case presentations as required.

Contribute to the development and implementation of clinical policies, procedures and guidelines

Promote the delivery of evidence-based care to all patients, recognising the limitations of the evidence allowing autonomous decisions to be made

The ACP must recognise their limitations and adhere to the NMC Standards/or equivalent, i.e. HPC and Guidelines, referring to senior medical staff if scope of practice is exceeded

Participate in the evaluation/ audit of clinical standards

Report clinical incidents in line with the policy

Comply with all relevant regulations and policies

Communication:

To undertake effective multidisciplinary liaison across the primary/secondary care interface by initiating appropriate referrals to and receiving patients from other agencies

Education & Training:
