Are you looking for a new challenge and keen to develop a service using advanced practice skills?
The Early Intervention Team (EIT) are looking for an ambitious, multitasker, experienced ACP to work with us in developing our admission prevention service. The role will be working in the community reviewing patients in a crisis working alongside: therapists, nurses, social workers and support workers.
Main duties of the job
The practitioner will be able to provide a holistic medical/ clinical assessment of patients, such as: post non- injury fall, reduced mobility, chest and urine infections, increased confusion and able to diagnose and prescribe to enhance the EIT intervention.
The team are working to develop the Urgent Community Response guidelines and support more admission avoidance work supporting the ambulance service and 111.
The postholder will also develop pathways and clinical links with primary and secondary care colleagues. You will work closely with a number of teams to develop new ways of working.
Full or part- time considered. The ACP service will run 7 days a week 8:30- 21:00.
