Job summary
We are seeking a Ward Manager for our newly established
Virtual Ward programme. This is an
exciting role providing clinical leadership to support management of patients
on the Virtual Ward and further development of the Virtual Ward model in line
with local and national strategy.
NHS England and NHS Improvement have appointed all
integrated care systems (ICSs) to extend or introduce for the first time the
virtual ward model with an ambition for all systems to deliver virtual ward
capacity equivalent to 40 50 virtual wards beds per 100,000 population
(equivalent to the delivery of up to 24,000 virtual ward beds), by December
2023.
Virtual wards have multi-professional input from
consultants, specialist nurses, advanced care practitioners, pharmacist and a
wealth of other health and social care staff.
Whilst we a looking for someone with experience of working with multi-professional
teams, and with strong leadership and communication skills, this provides candidates
with an exciting opportunity to develop new skills and help shape, develop, and
expand clinical pathways that directly benefit patient care.

Main duties of the job
The role of the virtual ward manager is to coordinate the
care of patients on the virtual ward pathways through efficient leadership and coordination
of the multidisciplinary team to provide safe, effective care to patients. The post holder will be expected to work collaboratively
with acute and community and services.
West Suffolk the Virtual Ward went live in late November 2022 and provides specialist led care for a variety of patients including those with respiratory conditions, frailty, acute kidney injury and heart failure. There is anticipated expansion into a few other areas during 2023/24 and a plan to increase both capacity and occupancy. This care is provided to them in the comfort of their own homes using an advanced personal remote monitoring platform, enabling them to safely leave hospital sooner or not come into hospital at all.
