Job summary

As a Case Manager on Ward 4, you play a pivotal role in coordinating patient care across complex specialties including Neurology, Rheumatology, Renal Medicine, and General Internal Medicine. This position involves working closely with multidisciplinary teams to ensure safe, efficient, and patient-centred discharge planning.

Key responsibilities include:

  • Acting as the primary point of contact for patients, families, and healthcare professionals regarding discharge and care transitions.
  • Assessing patient needs and developing tailored discharge plans that align with clinical recommendations and community resources.
  • Liaising with internal teams and external agencies to facilitate timely referrals and continuity of care.
  • Monitoring progress, addressing barriers to discharge, and escalating issues where necessary to maintain patient flow.
  • Supporting quality improvement initiatives and compliance with hospital policies and national standards.

This role requires excellent organisational skills, strong communication, and the ability to manage complex cases in a fast-paced environment. You will contribute significantly to improving patient outcomes and enhancing the overall experience of care.

Main duties of the job

  • Coordinate patient flow and discharge planning for complex cases across Neurology, Rheumatology, Renal, and General Internal Medicine specialties.
  • Use OPTICA to monitor patient status, track discharge readiness, and update care plans in real time.
  • Maintain and update the Ward Board daily to ensure accurate visibility of patient progress and discharge priorities for the multidisciplinary team.
  • Act as the key liaison between patients, families, clinical teams, and external agencies to facilitate safe and timely discharge.
  • Identify and address barriers to discharge, escalating issues where necessary to maintain patient flow and meet hospital targets.
  • Arrange referrals and community support services, ensuring continuity of care post-discharge.
  • Participate in board rounds and multidisciplinary meetings, providing updates and supporting decision-making.
  • Contribute to quality improvement initiatives, ensuring compliance with hospital policies and national standards.
