Diabetes Pre-Assessment Nurse

Job summary

This is an exciting opportunity to work as an integral member of the Pre Assessment Team, providing advice and support to patients who have diabetes and require surgery at James Paget University Hospital.

The perioperative diabetes pathway was implemented with Get It Right First Time to improve patient safety, reduce post-op complications and improve patient experience. Support is provided throughout the perioperative journey with the majority of patient contact occurring before surgery as outpatient telephone consultations and in-person clinic appointments such as diabetes medication/ Insulin starts, followed by Inpatient post-op. ward reviews when required.

This post is suitable for applicants with knowledge and clinical experience in caring for people with Diabetes.

The successful candidate will have excellent clinical, organisational and communication skills.Interest in education and enabling service improvement will be advantageous.

Main duties of the job

To support the work of the Diabetes Pre-Assessment team to provide clinical support and expertise to Diabetic patients within the outpatient departments.To provide a high standard of nursing care and to develop existing specialist Diabetes knowledge and skills.To act as a Diabetes nursing resource giving advice and psychological support to patients, carers and healthcare professionals.To review nursing practice and support the implementation of appropriate changes based on research/best practice.To assist with the development and implementation of audit, quality and Risk Management initiatives.To support the work of the Diabetes team; providing clinical support and expertise to the service.

To assist in the management of a clinical caseload as agreed and indicated by the team leader/manager for Diabetes.To provide clinical advice, education and support for patients, carers and other healthcare professionals in the treatment and management of diabetes and its complications.To ensure that agreed standards of nursing practice are maintained.To perform ongoing monitoring of Diabetic patients, under the direction of the Consultant Endocrinologist or medical physician.Actively support/enhance good communication between members of the multi- disciplinary team, the patient and their carers.To maintain accurate patient records and adhere to Trust Information Governance standards and policies.To actively participate in the collation of data for research and audit purposes.
