Job summary
Working as part of the children's nursing team, this role focuses on the provision of direct care to children and their families in an outpatient and clinic environment. The post holder will assist with service delivery, working in conjunction with the Consultant for rheumatology. They will be an advocate for children and their families, promoting child focussed, family-centred care and facilitate effective liaison with the multi-professional team involved with the child's care. The post holder will act as a role model in the delivery of patient care and support and teach staff contributing to the development of the service. They will identify specific needs in education of patients, relatives and staff and facilitate meeting these requirements while offering an individualised service that respects the many cultural and spiritual backgrounds of our families.

Main duties of the job
-To assist with the provision of a specialist, co-ordinated service, utilising nursing skills, expertise and knowledge in the care and management of children under the care of the rheumatology service
- Be able to support clinics, alongside the Consultant and other members of the multi-professional team, including physiotherapists and ophthalmologists, and communicate condition-related information to the child and family
- Takes an active role in clinical audit and be able to represent the service at governance meetings, disseminating relevant information to the wider multi-professional team.
- Able to deal with complex clinical facts and social situations which require analytical skills.
- To support the transition of young people from paediatric to adult care, liaising with the adult teams and transitional care specialist nurses, and supporting transition clinics in outpatients
- To facilitate the opportunities for department-based training which is dynamic and meets the educational needs of staff delivering care in a specific area
