Job summary
The Dementia Nurse/ Advisor will provide support and clinical knowledge to ward staff in relation to Dementia care management. Act as a resource to Trust staff and other professionals who deliver care to the patient. Ensure standards are in line with the NICE guidelines, National, regional and Trust Dementia Strategy.
Working days will be Wednesday, Thursday and Friday

Main duties of the job
The Dementia Nurse/ Advisor will support the completion of the Dementia screening and ensure that the Multidisciplinary teams understand the rationale for the screening in the Trust in line with the indicators highlighted in the previous National CQUIN. Support and complete holistic person-centred nursing assessments on patients within North West Anglia NHS Foundation Trust. Support the complex needs of carers through ongoing assessment of health and wellbeing working closely with the Trust Carers advisor, other partner organisations such as Age UK, Caring Together, Alzheimer's Society and the Admiral Nurse service. Provide training education/support to the Dementia Specialist Nurse and facilitate appropriate education/training on Dementia aspects of care. Work with the members of the Patient Safety Team to aim to provide harm free care for all patients.
