Job summary
We are currently seeking a highly organised and proactive individual to join our team as a Tracker. The primary purpose of this role is to manage the discharge and flow of inpatients. You will work close with the multidisciplinary team to ensure timely and effective discharges.

Main duties of the job
As a Tracker, you will work closely with both clinical and administrative staff, as well as other health and social care providers. Your main duties include monitoring, reporting, and tracking patients to ensure timely and efficient processing of their journey. You will resolve pathway delays promptly and escalate issues as needed.
