Job summary

This role of Transformation Manager will focus on the complex discharge processes at NWAngliaFT. The successful candidate will support internal NWAFT process improvement and provide a conduit to improvement where the process involves system partners and will be jointly supported by NWAngliaFT and the Home First Programme. The Strategy & Transformation Team (S&T) sits within the Corporate Division of North West Anglia Foundation Trust (NWAFT), under the management of the Deputy Chief Strategy and Transformation Officer.

The S&T team is currently recruiting towards an established long-term team, incorporating Strategy and continuous Improvement. It provides comprehensive support to, and assurance of, major improvement programmes of work across the trust. Ownership and responsibility for delivering these programmes sits within the Clinical Divisions, however the S&T team function is to support and accelerate improvement, enable change processes, working on continuous improvement to achieve these goals.

You will demonstrate effective programme and project management skills, with a credible background of successful improvement realisation, ideally in a health care setting. Knowledge of current project management techniques, methodologies, benchmarking and using best practice that can be translated and applied from the NHS and elsewhere is essential.

Main duties of the job

You will work to develop and embed a culture of continuous improvement within the teams they support, leading through role modelling best practice. Key relationships will need to be developed with clinical divisional operational, management and work stream leads to ensure that programmes are delivered smoothly and to a high standard, with clear objectives and realisable benefits and outcomes.

The main goals and objectives are as follows

Delivery of the restart and return operational process for NWAFT and system partners

Improvement actions to reduce the lost capacity.

Working with information services develop the discharge dashboard to enable accurate reporting and analysis of complex pathways and associated length of stay. This will include any benefits realisation from improvement work and data submission as required

Report progress and improvement data to the Home First Programme Board

Improve and simplify the current complex discharge processes at NWAFT , linking into improvement colleagues and stakeholders within the Back on Track Programme

Assess the E-Track operability and work alongside EPR colleagues to map tech processes. This may involve E-Track changes to maintain current operability

Support the enablement of a digital solution within NWAFT, liaising with digital/ tech colleagues and supporting/ training discharge colleagues/ wards on it use. Measure benefits.

Establish a stakeholder "huddle" to engage and rapidly improve.
