We are looking for someone who is an excellent communicator, enjoys problem solving and able to work by themselves and as part of a team to support and protect the welfare of anyone with a learning disability and/or autism within the Trust.
It is essential that you have up to date experience of caring for children, young people, adults at risk and their families where safeguarding issues have been identified.
You will be involved and responsible in ensuring children and adults at risk are discharged from the hospital in a safe and timely manner, that family receive outstanding care and that information is effectively communicated to a wide range of health and social care professional agencies.
You will have the opportunity to work closely with the Lead Professionals for Safeguarding Children and Adults to ensure that local, regional and national policy and guidance is reflected in our policies, guidance training and practice.
Main duties of the job
The Post Holder is expected to:
- Be an expert practitioner to support colleagues and acute hospital staff to meet the needs of people who have complex need through innovative provision of reasonable adjustments
- Communicate with multiple stakeholders across a range of organisations and departments to develop agreed plans and make best use of resources available
- Work within the acute Trust to provide expert guidance on the needs of people who have a learning disability and / or autism accessing acute settings to aid the development of pathways, procedures, and policies to provide equitable access
- Work collaboratively with the acute Trust on agreed initiatives to improve knowledge, skills and practice at supporting people who have a learning disability and / or autism (i.e. champions network, training, reasonable adjustments resources)
- Attend meetings (both internal and external), as agreed with the Lead for Learning Disability & Autism to promote access.
- Accept referrals and assess individuals needs in order to provide reasonable adjustments to facilitate equitable access for people who have a learning disability and / or autism across all Trust sites
- Ensure timely support by way of direct contact with patients with Learning Disability and / or Autism, their family, carers and / or advocates, as necessitated by clinical need and will liaise with all parties in a respectful and efficient manner
