Job summary

Are you a qualified RMN, RNLD, and Advanced Clinical Practitioner with the experience and passion to deliver outstanding, high-quality mental health care for older people?

We are seeking a skilled and motivated Advanced Clinical Practitioner (ACP) to join our Older People's Community Mental Health Team (CMHT). This is an exciting opportunity to work at an advanced level of practice, providing expert assessment, diagnosis, and management of complex mental and physical health needs in older adults.

As part of the Community Transformation Programme, you will play a pivotal role in delivering innovative, person-centred, and population-based care, with a strong focus on reducing health inequalities and improving outcomes for older people.

You will work autonomously for a significant proportion of your role, managing complex clinical presentations including frailty, comorbidities, and severe mental illness, while also contributing to leadership, service development, and clinical excellence across the service.

Main duties of the job

As an Advanced Clinical Practitioner, you will work across the four pillars of advanced practice:

Clinical Practice

  • Provide advanced clinical assessment, diagnosis, and treatment for older people with complex mental health needs
  • Manage complete episodes of care independently, from initial assessment through to discharge or ongoing management
  • Demonstrate expert clinical reasoning in managing comorbid mental and physical health conditions, including frailty
  • Prescribe independently (where applicable) and initiate appropriate intervention

Leadership & Management

  • Act as a senior clinical leader within the CMHT
  • Contribute to service transformation, development, and delivery of high-quality care
  • Support and guide multidisciplinary team members in complex decision-making

Education

  • Provide clinical supervision, mentorship, and training to staff and students
  • Contribute to the development of a positive learning culture within the service

Research & Quality Improvement

  • Apply and promote evidence-based practice (e.g. NICE guidance)
  • Lead and participate in audit, service evaluation, and quality improvement initiatives
  • Contribute to clinical governance and continuous service improvement

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country.
