We are seeking a motivated and enthusiastic Care Coordinator to join our Working Age Community Mental Health Service. This is an excellent opportunity for someone committed to supporting service users in their recovery within a community setting.
Our Community Mental Health Teams operate Monday to Friday, 9am-5pm, and we offer hybrid and flexible working arrangements to support a positive work-life balance.
As a key member of our multidisciplinary team, you will provide a range of evidence-based interventions for working-age adults experiencing mental health difficulties. We are looking for individuals who are organised, proactive, and able to work both independently and collaboratively. You will bring strong risk assessment skills, a recovery-focused approach, and a solid grounding in therapeutic interventions. Central to your role will be working with service users to ensure their recovery goals inform their formulation and care planning.
This is an exciting time to be part of our service. We are preparing for the Community Transformation Programme, a national initiative aimed at enhancing mental health care for people with severe and enduring mental illness (SMI).
Main duties of the job
You will have the unique opportunity to work alongside experts by experience, third-sector partners, primary care, and adult social care, contributing to service improvements that address health inequalities and better meet the needs of our communities. This collaborative and forward-thinking approach makes the role both refreshing and highly rewarding.
With access to appropriate clinical and management supervision the post holder will have or will develop and will be able to demonstrate in practice a range of appropriate skills to include assessment (including risk assessment), formulation, safety planning and clinical interventions.
The role provides opportunity to coordinate the care of service users, throughout their engagement with the service and wider services if these are required. To achieve this the post holder, through a range of defined competencies, will acquire all of the skills necessary to carry out role. This will include a requirement to practice care co-ordination skills through shadowing opportunities and supervision. Workload will be continually negotiated through supervision and case load management with the appropriate clinical team managers.
