Job summary

The Forensic Outreach & Liaison (FOLs) Team for North Yorkshire is based at Clifton House, and supports service users throughout York, Harrogate, Scarborough, Selby & surrounding areas. We work closely with medical, psychology and social work colleagues to safely support service users make the transition from secure inpatient care to community settings, working alongside partner agencies in the management of complex risk in community settings. We care co-ordinate, joint manage and repatriate patients both on our caseload, in transition and from out of area ensuring they remain in the place of least restriction.

Main duties of the job

The role of the community forensic nurse will be to assist with the transition of patients from secure services into the community in a timely and safe manner. This will involve care co-ordination and co-working alongside other partner agencies.

As a member of FOLS, using an MDT approach you will assist in managing referrals received for consultation from generic CMHT's, where concern has been raised in relation to an individual's risk to others. This may involve providing specialist risk assessments to assist with the management of these service users.

We are looking for a dynamic, motivated and experienced mental health nurse to take on the Band 6 role. The ideal candidate will be thorough, enthusiastic, emotionally resilient, and able to work autonomously with individuals in this transitional phase of their care pathway. Previous experience of community work would be an advantage but not essential as you will receive a thorough induction and training to support your development. An ability to work as part of a team and support your colleagues is essential.

The successful candidate must have current NMC registration, a full UK driving licence, and access to a vehicle for work use.
