Job responsibilities

The role will involve working with individuals with a complex psychosis from assessment of rehabilitation needs, through care planning interventions to build on skills and increase independence and developing safety and risk management plans for individuals in the community.

You will coordinate the care of a caseload of individuals, whilst working alongside a diverse MDT supporting delivery of a range of interventions to support the rehabilitation of this population both across the community, but also those who require inpatient rehabilitation, or an episode of acute care.

In addition to direct patient care there will also be coordination of the shift through a duty worker role ensuring safe and effective care, management of lone working and prioritisation of clinic care across the team through a duty worker system.
