Job summary

We have an exciting opportunity for an enthusiastic and motivated Pharmacy Technician to join our progressive Medicines Optimisation team.

We're looking for a dedicated, proactive individual who brings strong organisational, interpersonal, and time management skills. You should be a confident team player who's equally comfortable working independently, and passionate about making a difference in the lives of people with mental illness and learning disabilities.

Main duties of the job

The successful candidate will work closely with the Pharmacists and all members of the MDT to co-coordinate all aspects of the patients' medicines related journey, communicating effectively to ensure a safe, seamless and integrated transfer of care for our patients.

Key areas of responsibility include:

Completion of medicines history/ reconciliation when patients are admitted into our care.

Deliver a patient focused medicines optimisation service to clinical areas.

Ensure the timely supply, storage and management of medicines within inpatient areas.

Provide information to patients and carers about their medicines and support adherence and understanding.

Support with transfer of care into primary care, utilising associated systems, liaising with other healthcare professionals.

- Provide essential technician services, including checking patients' own medication, conducting self-medication assessments, and assisting with the dispensing and supply of medicines.

- Participate in the training and supervision of new staff, trainees, and wider healthcare team.
