We are looking to recruit a highly motivated individual to join our Social Worker team at Clifton House low secure hospital in York. Working across 3 inpatient wards and multidisciplinary teams, you will have a key role in contributing to service user pathways from admission to discharge, with a particular focus on discharge planning and risk management. You will liaise with a range of external agencies including Local Authority Social Services, Police, Probation, ICB's and Housing Providers to ensure a smooth and timely transition out of secure services.
The Social Work Team is going through a period of development and this is an exciting opportunity for you to participate and provide ideas of how the Social Work provision can be enhanced to maximise user and carer involvement whilst maintaining an MDT approach to managing risk as well as identifying and working towards appropriate discharge pathways.
Main duties of the job
To work within a multidisciplinary team to provide appropriate assessments and deliver interventions to individuals, families and carers in order to support recovery, promote self-management, and minimise risk.
To undertake duties under the Mental Health Act as required e.g. report writing and attendance at Mental Health tribunals and Hospital Managers Hearings.
To undertake statutory work as defined under the Mental Health Act 1983 and Mental Capacity Act and demonstrate an understanding of the restrictions that legal orders may have on a patient's life.
To undertake complex case work involving risk management, and a working understanding of safeguarding processes and responsibilities.
To work closely with patients and the multidisciplinary team to ensure timely discharge planning alongside Section117 aftercare service support. Liaison with Local Authorities and with housing providers / placements.
To take on the role of Carer Lead for the hospital. Supporting families and carers, supporting the Ward Carer Champions, making links with community organisations,
To seek to empower service users, and promote anti discriminatory practice.
