Job summary

We are a blended team of lived and occupational experts, with a focus on enabling progressive and trauma-informed journeys through secure care. Our aim is to help improve the experience and outcomes for individuals who experience the world as profoundly unsafe (often associated with the diagnostic label of a 'personality disorder'). We work alongside inpatient and community teams for adults and young people (16 +) on a secure care pathway across the region or at risk of entering secure care.

Changes following the introduction of Integrated Care Services (ICSs) mean that this is an exciting time to be joining the team. We operate across the region covered by the three Provider Collaboratives (PCs) that serve the population of West Yorkshire, South Yorkshire and Humber & North Yorkshire ICB.

As an award winning, innovative service we can offer real opportunities to be involved in improving the experience of service users across these pathways. This is a great opportunity for an experienced mental health practitioner with a keen interest in personality disorder who would be interested in developing best practice and contributing to the development of local and regional pathways.

You will need to be qualified in nursing, social work or occupational therapy, with post qualification experience, or be able to demonstrate significant experience of working with this service user group.

Main duties of the job

As a Caseworker within the PDS you will be involved in working alongside teams to learn about what has happened within a system to obstruct pathways for their service users. This may involve:

  • undertaking reviews which seek to develop the understanding of an individual's needs, making key best practice recommendations to their responsible team.
  • support to teams to implement, review and adapt recommendations from existing reviews of care.
  • collaborate with clinical teams to create 'thinking spaces' which aim to foster conditions for something different to happen in their service user's care pathway.
  • support systems and teams striving to deliver trauma - informed, integrated care to service users who are either at risk of entering secure hospital or presently receiving treatment within secure care.
  • delivery of training and consultation.

The Pathway Development Service actively promotes and maintains a reflective, safe and supportive culture. You will be provided with a comprehensive package of training and development opportunities, and access to both regular clinical and managerial supervision.

Prior experience of working within a secure or specialised inpatient pathway or team would be desirable. This role will provide opportunity to build expertise and knowledge in the field of personality disorder / trauma informed care, gain a range of valuable skills in the area of complex care reviews, supporting specialist care systems, best practice and inpatient care pathways.
