Job summary

We are currently seeking a dedicated Harm Reduction Practitioner to join our dynamic team and contribute to our mission of providing compassionate care and support.
Ward 5, located at the Newsam Centre in Leeds, is a 17 bedded locked complex rehabilitation service that delivers rehabilitation and recovery to male service users with enduring mental illness. The service provides a holistic, psychologically informed approach to support service users in developing their life skills, independence and hope for the future in a way that is meaningful to them, in the least restrictive way possible, with a focus on individual strengths.
The job role will include providing specialist addiction treatment, including psychosocial and pharmacological interventions, harm reduction, & risk management to service users with co-existing mental health needs. The Ward 5 Team is trauma-informed and offers a formulation driven, psychological model.
The successful candidate will receive excellent supervision from both the ward and the COMHAD (Co-Occurring, Mental Health, Alcohol and Drugs Team) at Forward Leeds.
Main duties of the job
As a Harm Reduction Practitioner, you will:
- Provide specialised treatment and support to service users with complex mental health and substance misuse needs.
- Deliver expert psychosocial interventions and offer harm reduction advice tailored to individual needs.
- Develop and implement group work programs designed to address the unique requirements of designated service users.
- Collaborate closely with the Multidisciplinary Team, other teams, families, and carers to assess addiction issues and contribute to individual care plans, clinical risk assessments, and safety plans.
- Offer specialist advice and supervision to the wider multidisciplinary team regarding addiction.
- Work collaboratively with operational, clinical, and professional leads to identify and address addiction training needs, developing and delivering workplace learning and educational activities.
