Job summary

Forward Leeds is an integrated Drug and Alcohol service made up of several key partners including Leeds & York partnership Foundation Trust. We are a trauma informed service, providing a variety of specialist psychosocial interventions and formulation for people who have co-occurring mental health and substance misuse problems.

The role of the mental health professional will be to work as part of a wider, established team to minimise and prevent rough sleeping and begging within the local authority of Leeds by engaging and maintaining clients into treatment and support individuals to move into supported, more permanent accommodation.

To provide mental health and addiction support to adult service users and to coordinate their drug treatment, including assessment, recovery coordination, specialist psychosocial Interventions (SPSI) delivery and multi-agency working.

To build and maintain links with secondary mental health service in Leeds, particularly the Crisis service, so as to improve access to treatment for this client group.

The team will be based in the city centre and will go out onto the streets of the city to offer help and support to those living street-based lifestyles.

This newly developed team will have the time and space to deliver care directly to those service users using a trauma-informed approach.

Working collaboratively with the city's other street support teams, we hope to be able to provide exceptional holistic treatment to those who need it most. Treatment services in Leeds are unique and comprehensive and this team has a real opportunity to make a huge difference to those that needs support the most.

Job responsibilities

To work in a proactive role to minimise and prevent rough sleeping within the local authority of Leeds. To support individuals to move into supported, more permanent accommodation; support their sustained commitment to stay accommodated, engagement with local services and improved health and wellbeing.

The post holder will actively use their knowledge, skills and experience to support the Clinical Leads, Lead Consultant & Clinical Operations Manager in achieving all service standards set for the team.

Key Activities

  • Co-ordinate and lead a trauma informed focussed approach to service users identified as vulnerable, who have experienced trauma and are rough sleeping, with a view to engaging them in appropriate services and move them away from street based lifestyles.
  • To participate in patterns of shift work to maximise potential contact with service users through assertive outreach sessions.
  • To work in close partnership with CGL street outreach team and Street Support teams to assist service users with mental health difficulties to gain rapid access to advice/support/ treatment.
  • Attend the Multi-Agency Review Board (MARB) to support coordination, partnership working and support of service users.
  • To work alongside already established street outreach teams in Leeds, working specifically with rough sleepers in order to develop positive relationships and work with them on their mental health and past trauma.
  • To carry a small caseload of complex service users
  • To create and review robust risk assessments and care plans.
  • To support service users in accessing information and services relating to their lifestyle and personal choices.
  • To participate in multi-agency initiatives and shared working to reduce the level of street activity and its associated impact.
  • To identify and target entrenched rough sleepers/beggars, including those who find engaging with service very difficult.
  • To maintain comprehensive records on service users, updating case notes and databases in a timely and accurate way.
  • To assess the needs of the street population and involve specialist agencies, working closely with those services to achieve outcomes for service users.
  • To maintain service user confidentiality compatible with the effective sharing of information with appropriate agencies to achieve the best outcome for service users
