Job summary
We are looking for a friendly, enthusiastic, and motivated individual with a passion for supporting those with mental illness and learning disabilities to join our pharmacy team within LYPFT. The successful applicant will have excellent communication, organisational and time management skills who can work effectively as part of a team and independently.
Main duties of the job
The successful applicant will work closely with our clinical teams and Pharmacists in providing a high-quality medicines optimisation service to clinical areas within LYPFT. Key areas of responsibility include:
- Completion of medicines reconciliation
- Ensuring timely supply of medicines
- Delivering a patient focused medicines optimisation service to clinical areas.
- Support the timely and smooth transfer of patient care at the point of discharge.
- Participate in the training and supervision of new staff, trainees, and wider healthcare team

Job responsibilities
- The post holder will have direct communication with other pharmacy personnel, prescribing doctors, nursing staff, service users and carers.
- Take detailed and accurate medication histories and check that the medicines prescribed for each patient correspond with the information gathered and ensure that this is documented within the relevant electronic system(s)
- The post holder will communicatecomplexand sometimes sensitive medicines-relatedinformation to a variety of healthcare professionals, collectively and individually.The post holder may have to adjust their communication styles and content to take into accountother peoples language difficulties, intellectual abilities or background, for example.
- The post holder will ensure theyrecord all their interventions made during their working week as agreed by their manager.
- Provide information (verbal and/or written) to service users with regards to the safe and effective use of their medication, prescription payments, exemptions, and further supplies.
- To liaise withprimary, secondary, and tertiary care services (GPs,CPNs social workers,community pharmacistsetc.), to improve the pharmaceutical care of patientsunder the care of community services withinLeeds.
- To liaise with other healthcare professionals in the provision of a pharmaceutical service to a client group which includes some with very specific needs and difficulties and others who may exhibit severe challenging behaviour.
- Developing relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care and mental health.
