This is an opportunity for a Band 6 Occupational Therapist to work in a recovery orientated Community Rehabilitation Enhanced Support Team (CREST).

The service is a regional service working across the West Yorkshire Integrated Care System (Bradford, Calderdale, Kirklees, Leeds and Wakefield)

CREST is focused on providing different ways of working with individuals who have spent long periods of time in hospital. The service is dedicated to providing psychologically informed, flexible, individualised packages of care to build service user confidence to live a meaningful life in the community, as an alternative to a hospital complex rehabilitation placement.

As part of expanding our Occupational Therapy workforce we are looking to recruit 2 Band 6 OTs, one for our expanding Serious Mental Illness (SMI) pathway and one for our developing Complex Emotional Needs (CEN) pathway.

In addition to the expanding OT provision within CREST there will also be the opportunity to connect with other occupational therapists across the service.

Main duties of the job

The role will involve working closely with the OT lead, establishing new ways of working in the community. The job will focus on identifying, assessing, planning and carrying out interventions related to a service users rehabilitation needs. There is no care co-ordination within this role.

The post holder will work alongside the wider MDT team to assess individuals and identify occupational need, barriers to discharge, community support needs and identifying interventions and care plans which can support the transition of service users stepping down to a less restrictive environment.

We are looking to expand our team with individuals who share our passion for promoting recovery and supporting transition from locked rehabilitation environments into less restrictive environments. CREST work using a team approach to deliver care interventions which have been identified through the assessment process which is supported by a diverse multi-disciplinary team which includes peer support, medics, psychologists, occupational therapists and nurses.

Due to the nature of the service user group there is a need for high levels of multi-agency working to ensure effective communication and maintain high levels of care being delivered to service users on their recovery journey. Liaison with supported housing and other NHS and third sector organisations is something that the successful candidate will be required to undertake on a regular basis.

The postholder will have a good working knowledge of the principles of recovery and will practise clinically in a way that encourages therapeutic optimism and hope with service users, and work collaboratively to promote independence.

The service has a strong commitment to co-production and work closely with co-production colleagues in engaging service users in the development of the service.

You will have access to profession specific supervision, line management supervision and reflective practice. We will also support in continuing professional development.

We are looking for someone who can inspire others, create hope and make positive connections to help people build a meaningful life in the community. You will make a real difference to people placed in complex (locked) rehabilitation hospitals by providing an alternative offer as part of a developing regional community team. You will be at the forefront of developing the Occupation Therapy offer, utilising contemporary Occupational Therapy interventions to help people work towards a meaningful life in the community.

Supporting discharge from hospital requires flexible and intensive transitional community support closer to home, in the least restrictive environment, providing consistent, recovery focussed and contemporary interventions and integrated pathways of care. Being multi-disciplinary and multi-agency in nature will create psychologically informed flexible care packages to support discharge home.

The Complex Rehabilitation project is part of the West Yorkshire Integrated Care System (WYICS) Programme for Mental Health and Learning Disability. Consultation with service users, carers, commissioners and mental health providers has highlighted the service user experience and challenges for people placed in mental health complex (locked) rehabilitation placements from West Yorkshire.

Many have experienced long lengths of stay in locked hospital placements, are disconnected from a community. Service users have shared very powerful experiences with us, describing a community model as not just about keeping me safe its about building a life worth living.

We recognise the importance of ensuring that our services are reflective of the communities we serve and champion diversity and inclusion.
