Job summary
We are looking for a friendly, enthusiastic, and motivated individual with excellent communication and organization skills to join our pharmacy team within LYPFT.
Main duties of the job
The successful applicant will work closely with our clinical teams and Pharmacists in providing a high-quality medicines optimisation service. Key areas of responsibility include:
- Completion of medicines reconciliation
- Deliver a patient focused medicines management service to clinical areas.
- Support the timely and smooth transfer of patient care at the point of discharge.
- Participate in the training and supervision of new staff, trainees, and wider healthcare team.
