Job summary
We are looking for an enthusiastic and motivated band 5 Occupational Therapist to join our dementia in -patient service to work on either ward 1 or ward 2 at the Mount. Ward 1 is a 14 bedded male dementia ward and ward 2 is a fifteen bedded female dementia ward both which offer person centred dementia care for service users in Leeds
You would be working alongside the Band 6 OT and our Occupational Therapy Assistant as well as the rest of the MDT team.
Main duties of the job
Main duties would be the person centred occupational assessment of individual service users need. Includes assessment from holistic view point. Planning and implementation dementia care specific occupational / occupation focused groups. The person should be able to review and update service users care plan and review in partnership with other MDT members. Managing a case load of service users with a diagnosis of dementia using effective communication with MDT members, patients and carers.
