Job summary

Durrant Ward are looking for a dynamic forward thinking Discharge Co-ordinator to join their team. Durrant ward is a 31 bedded inpatient unit specialising in frailty.

The post is 37.5 hours per week, Monday to Friday.

Main duties of the job

To facilitate safe, effective and timely discharge from hospital by working in collaboration with other health/social care professionals.

To monitor for any delays in discharge and act efficiently to solve and escalate them.

To assist flow through the hospital and create capacity.
