An opportunity has arisen for registered nurses to join our highly motivated, operational discharge team as a discharge lounge nurse.
The Discharge lounge supports all areas within the trust with an emphasis on maintaining timely and safe patient flow out of the hospital.
This year the hospital discharge lounge is a purpose built ward providing an improved facility to support better care to all of our patients when they are ready for discharge.
The discharge lounge offers opportunity for Band 5 nurses to further develop their knowledge and skills within a fully supported environment. With an aspiration to improve and support every patient journey, across elective and non-elective pathways, this provides opportunity to nurses across all specialties to utilize their skills. The discharge lounge co-ordinates a MDT approach to discharge including Hospital wards, Pharmacy, Therapists, Social workers, transport agencies etc.

DISCHARGE LOUNGE BAND 5 Nurse
The Operational Discharge lounge offers an exciting opportunity to join the team as a permanent Band 5 nurse.
The discharge lounge is an evolving busy unit that supports all trust areas to provide a safe and timely discharge from the hospital. The discharge lounge co-ordinates a MDT approach to discharge including Hospital wards, Pharmacy, Therapists, Social workers, transport agencies etc.
The successful candidate will have experience of acute hospital care and a knowledge of patient discharge processes.
The band 5 role requires the individual to have enthusiasm and a passion to drive the service forward and meet service demands. They should have effective communication skills and an ability to support all the staff within the team to deliver high quality safe patient care.
The successful applicant must be flexible as the role will require working a rota including night and weekend shifts. They will have an understanding and experience supervising a team of health workers as it will be an expectation that you will be shift lead at times.

The post holder must be able to work without direct supervision, ensuring patients are transferred out of the hospital in a safe and timely manner.
- The post holder will need to be flexible and carry out nursing duties in accordance with the NMC Code of Professional Conduct and ensure all Professional Standards and Trust Policies are complied with.
- The post holder will assist senior staff with optimising operational patient flow throughout the hospital.
- The post holder will use a holistic patient centred approach to achieve objectives and provide support to relatives/carers whilst supporting patients through their discharge journey.
