Call Handler Transfer of Care Hub
37.5 hours per week
We are looking for a motivated and organised Service Liaison Assistant to join our busy Transfer of Care Hub to cover maternity leave
As the first point of contact, you will handle incoming calls, manage referrals, and ensure accurate information is recorded to support safe and timely transfers of care. You will work closely with clinical teams, community services, and external partners in a fast-paced environment where attention to detail and excellent communication skills are essential.
This is an exciting opportunity for someone with strong customer service or administrative experience, ideally within a healthcare setting, who thrives under pressure and is passionate about delivering high-quality support.

Main duties of the job
To update patient records in line with data protection guidelines while handling a high volume of calls and email referrals from patients, relatives, and healthcare professionals. Your role will involve screening enquiries, gathering key information, and directing staff/stakeholders an patients to the appropriate services. You will process and prioritise referrals, ensure patients are eligible for services, and coordinate communication between patients, GPs, and community teams.
You also work to service level agreements, support hospital discharge coordination (including transport), monitor systems such as health alert portals, and respond promptly to urgent issues. In addition, you will contribute to team effectiveness by training new staff, participating in meetings, and maintaining strong working relationships, all while managing exposure to sensitive and sometimes distressing situations.
