Job summary

Full time - 37.5 hours per week

Fixed Term Contract - 9 months Maternity Cover.

The post holder will provide daily supervision and support to the Out of Hours community nursing team to ensure the provision of high quality, evidence-based nursing care to patients within a community setting.The post holder will be responsible for assessing, planning, implementing, and evaluating care, particularly for patients with more complex needs in the Out of Hours service team.The post holder will deputise in their absence of the Line Manager, in maintaining a safe environment, whilst utilising the staff team to maximum efficiencyThe post holder will provide structured clinical supervision and mentorship to junior staff and students, fostering professional development and reflective practice.

Main duties of the job

  • Co ordinate and lead daily care in the community Out of Hours service of an assigned group of patients and staff, ensuring high standards of clinical practice
  • Complete Clinical triage so that schedule care is prioritised dependant on the patient's needs and clinical condition.
  • Utilise analytical and judgement skills to carry out clinical risk assessments within scope of competence, escalating concerns related to poor clinical standards or clinical risk.
  • Communicate complex and often distressing information clearly and compassionately, including end of life and safeguarding matters to patients and/or their families.
  • Communicate effectively with patient's and relatives ensuring that they are informed and kept up to date about clinical care and visits.
  • Be a role model for high standard, compassionate, inclusive care.
  • Combine autonomous practice with day-to-day supervision of the nursing team, ensuring personal responsibilities are fulfilled while supporting others. Ensure safe, effective, and coordinated care delivery through guidance, delegation, and collaborative problem-solving.
  • Provide regular staff 1:1's, sickness and absence monitoring, Personal Development reviews, identifying staff individual performance concerns and provide constructive feedback to a allocated group of team members.
